Head into the Temple and kill humans and golems until you collect all 7 Parchment Scrap, they are lore so easy to keep track of. Return to Cadmael and turn them all in to get a Completed Cultist Cipher and a Reconstructed Cultist Manual. There are 3 possible manuals and which you get is random:

Head back into the Temple and make your way to the large north room. Clear yourself some room, stand on the skull insignia on the floor, clear your target, and /say the line below which matches the Manual you were given:

1) May an unholy blight wash across these lands The boon of disease shall open the path for the return of the Lord of Pestilence
2) We foul our bodies with the blight we worship the Disease Lord with our flesh All hail Bertoxxulous the Unclean One
3) Bertoxxulous blesses us with corruption We call upon the Disease Lord to send a herald of divine pestilence

Vaxztn, a golem, spawns and attacks. There is about a 5 minute cool down before another player can summon if you are doing this for a group. Once Vaxztn is defeated return to Cadmael to complete the task.
